What is Primary Care?
Primary care providers (PCPs) are health care providers that you see throughout your life. Getting to know your primary care provider helps you trust them. When your provider knows your history, your likes and dislikes, and your goals, both of you make better decisions about your health.
Primary care focuses on preventing illness, so we ask that you or your child come for a visit every year – even if you don’t feel sick! Your provider might ask about your health concerns, run tests, and discuss ways that you can live a healthier life. For children, they also make sure they’re up to date on all recommended vaccinations.
Regular primary care visits help you catch health issues before they become an emergency!
Your Medical Home
We put our patients first. That’s why the National Committee for Quality Assurance (NCQA) recognizes Charlotte Community Health Clinic as a Patient Centered Medical Home (PCMH). Research shows PCMHs improve care quality, patient experiences, and staff satisfaction, all while reducing the cost of health care.
As a Patient-Centered Medical Home, we provide care that is…
- Comprehensive. We take care of most of your physical and mental health care needs, including disease management, acute care, prevention, and behavioral health. Our diverse team of people, some who work at CCHC and some who work for other organizations, ensures that we meet you and your family’s needs.
- Patient Centered. Our staff get to know you as a whole person. You are the expert on your health, and we respect your unique choices, values, culture, and preferences. We include you in making a care plan that works with your life and your family.
- Coordinated. Sometimes you or your family needs care that we can’t provide in our office. We work with specialty physicians, hospitals, home health services, and community support organizations to make sure all these providers communicate with each other.
- Accessible. Services like our triage line, our after-hours telephone service, and our MyChart patient portal app help you communicate with your care team whenever you need to.
- High Quality and Safe. We constantly improve how we care for you and your family by measuring the outcomes of what we do and incorporating this information into our practice. We base our clinical decisions on evidence and research.
