COVID-19 Update: CCHC is committed to continue providing high-quality care to all of our patients throughout the coronavirus pandemic. To keep everyone safe, we cannot accept walk-in patients right now. Patients who do not have an appointment will not be seen. If you are a CCHC patient, please call ahead or use your MyChart patient portal to make an appointment.

How to Pay for Your Visit

Affordable Health Care for Every Patient

To make sure that everyone can access high-quality health care, we offer many different ways to pay for your visit. We accept Medicaid, Medicare, and most major private insurance plans. We also offer a sliding scale discount program for patients who do not have health insurance.

Accepted Insurances

Not every provider may accept every insurance plan, and not every insurance plan covers every service. Please check with your insurance provider before your visit if you have questions about covered services and/or providers.

Sliding Scale Discount Program

CCHC offers sliding scale discounts to patients who do not have health insurance. Your copay, or the amount that you pay at each visit, depends on your household size and income. Copays for medical and counseling visits will range from $25 all the way up to the full cost of the visit. Copays for dental visits depends on the services that you receive. Our staff will let you know what your copay will be before your appointment. You need to turn in a Renewal Application once a year to keep receiving discounts on your visits.

The Sliding Scale Discount Program Application is included in our New Patient Application or Renewal Application on our Become a Patient page.
