Please fill out and turn in a New Patient Application available on our Become a Patient page. Once we process your application, our scheduling staff will call you to schedule your first appointment.
Yes. Please fill out and turn in the New Patient Application on our Become a Patient page. On the application, there is a place to choose whether you’re interested in medical, dental, and/or behavioral health services.
Once our staff process your paperwork, they will call you to schedule your first appointment. The most common reason that people do not hear from us is that staff are unable to reach you by phone or leave a voicemail. Please make sure that your voicemail is set up and has space to leave messages.
If it’s been more than a month since you turned in your application and you have not heard from us, please call our main number at (704) 316-6561 to check on your application.
We process new patient applications in the order that we receive them. We recommend that you turn in your new patient paperwork as soon as possible, but please recognize that the process may still take several weeks.

Our University clinic is open Mondays, Wednesdays, Thursdays, and Fridays 8am-5pm, Tuesdays 9am-5pm. Our West clinic is open Monday – Friday, 8 am – 5 pm. Our Dental clinic is open Monday – Thursday, 8:30 am – 4:30 pm. CCHC Express Care is open Mondays 8am-5pm, Wednesdays 8am-5pm, and Saturdays 8am-1pm. Yes. If you are a CCHC patient, you can make an appointment by calling (704) 316-6561 or using our Scheduling Form. You can request an appointment through yourMyChart Patient Portal. If you are a new patient, please fill out and turn in the New Patient Application on our Become a Patient page. Once we process your application, our scheduling staff will call you to schedule your first appointment.
Our Express Care location is open Saturdays 8am-1pm. If you need help when the clinic is closed, call our main number at (704) 316-6561 to speak to a nurse.

Often, your doctor will be helping other patients and can’t answer the phone. You can use your MyChart Patient Portal to send a message directly to your doctor. They will answer your message as soon as they’re able. You can also call our main number at (704) 316-6561 and choose the option to leave a message for your doctor’s medical assistant.
If you need help when the clinic is closed, call our main number at (704) 316-6561 to speak to a nurse. They can connect you with an on-call provider if necessary.
Insurance and Sliding scale
We accept cash, debit cards, and credit cards (Visa and Mastercard).
We accept many private/commercial health insurance plans, Medicaid, and Medicare. See more information on our Payment Options page.
Not anymore. In 2016, we became a Community Health Center, also known as a Federally Qualified Health Center (FQHC). We do charge for our services and offer discounted costs for patients who don’t have health insurance based on family size and income. We accept patients who have Medicaid, Medicare, and many private insurance plans, as well as patients who do not have health insurance. See more information on our Payment Options page.
Your copay, or the amount that you pay for your visit, depends on your household size and income. Copays range from $25 to the full cost of the visit. After you turn in the New Patient Application, our staff will contact you to schedule your first appointment and let you know what your copay will be. You can Become a Patient at CCHC whether or not you have health insurance. You may also qualify to buy affordable health insurance through the Health Insurance Marketplace, even if it’s outside of the usual Open Enrollment period. We need two pieces of information: the amount of money earned in your household and the number of people who live in your household. Everyone who lives in your home who are pooling financial resources including room and board and/or are supporting one another financially count as one household.
To demonstrate income, we need ONE of the following documents for each adult listed on the application:
- Most recent Federal Income Tax Return
- W-2 forms
- 30 days of your most recent pay stubs
- Letter from your employer on letterhead (must include contact name and phone number)
- Letter from the Social Security Administration, Veterans Administration or Social Service Agency indicating income
- Paperwork from the Employment Securities Commission (ESC) proving unemployment status and the amount of unemployment compensation being received.
- Alimony or Child Support Agreement
- Bank statement that shows last 30 days of income direct deposits
- Paperwork documenting retirement, disability, SSI benefits
- If homeless, a letter from shelter where you receive services
- If homeless or doubling up, a Doubling Up Verification Form
- If supported by a friend/relative, a signed Letter of Support from your friend/relative
To demonstrate household size, we need the following for each person listed on your application:
- A picture ID for all adults, such as a driver’s license, passport, or permanent resident card
- A proper ID for all children under 18 years old, such as a school ID, insurance card, social security card, or birth certificate

Yes, anyone can apply. Not everyone will qualify for a discount, as discounts are based on family size and household income.

During the appointment, an application counselor will help you determine whether you’re eligible for different kinds of insurance. If you are eligible, they will help you apply for insurance coverage.

Yes, we welcome all patients, regardless of where they live. Our discount program is not restricted to Mecklenburg County.
Both clinics are able to perform simple lab tests within the clinic. Our University clinic also shares a building with lab and imaging facilities. Your provider may send you to these facilities for lab or imaging tests. Our West clinic does not have a lab on-site, so we will take the samples we need and send them to an off-site lab for processing.
Please call our office at (704) 316-6561 and select the option for medical records. You will need to fill out a Medical Records Release form and turn it in to the clinic. We can send your records to you or send them directly to another business or doctor’s office.
Yes, we provide dental care for children and adults. The process to become a dental patient is the same process that is described on our Become a Patient page.
Yes, during flu season. For uninsured patients, a flu shot is included with your office visit or $15 as a walk-in.
We do not provide prenatal care, but we do refer patients who are pregnant to local OBGYN offices. We do provide some gynecological services such as pap smears, contraception, and STD testing and treatment.
We do have some specialty providers who volunteer on-site at the clinic. If you need care that we cannot provide on-site, we will refer you to a specialist. Our partnership with Care Ring’s Physician Reach Out (PRO) program may be able to help uninsured patients with the cost of specialty care.
Our medical providers can help with common mental health conditions like depression, anxiety, or substance use disorders. Our licensed clinical social workers also offer counseling for kids and adults to help learn tools to deal with stress and other emotional challenges. More complicated or severe mental health conditions may be referred to a specialist.
Prescriptions
At this time, we do not have an on-site pharmacy. We will ask you which pharmacy you prefer to get your prescriptions from, and we will send the prescription to that pharmacy.
Call your pharmacy. If you have refills remaining, they will fill the prescription and you can pick it up there. If you do not have refills remaining, they will contact us to obtain additional refills. Sometimes your provider may want to see you before refilling the prescription. If that’s the case, we will contact you to schedule an appointment.
Our providers work with you to make sure that you can afford your prescriptions. They may prescribe you a different medication that costs less. Your provider may suggest that you buy your prescriptions at a specific pharmacy or give you a coupon to reduce the price. They may also enroll you in a program called NC MedAssist that will help you get the prescriptions you need.
