We are always looking for helpful hands to chip in around the clinic or at community outreach events.
How do I volunteer?
All volunteers must complete a volunteer signup form, a volunteer waiver form, and a volunteer confidentiality agreement. Follow the links below to find online versions of these applications:

Important information for medical professional volunteers
If you are a medical professional looking to volunteer with us, great! We just need some additional information to complete our privileging and credentialing process before you are able to put your medical expertise into action.
All medical professional volunteers should let us know which services they are able to offer. To do so, volunteers are required to complete our Privileging form corresponding to their position (in addition to the volunteer application and waiver above). Click the link for your title below to complete this form.
CCHC Privileging – Nurse or Medical Assistant
CCHC Privileging – Primary Care Physician or Nurse Practitioner
CCHC Privileging – Clinical Social Worker
CCHC Privileging – Dentist or Dental Hygienist
CCHC Privileging – Chiropractor
CCHC Privileging – Clinical Pharmacist
CCHC Privileging – Obstetrician, Gynecologist, or Nurse Practitioner/Mid-Wife
CCHC Privileging – Registered Dietician or Certified Nurse Specialist
Additionally, all volunteer providers (e.g. MD, FNP) must complete our Credential Verification form. You can find it here (ONLY providers must complete this form):
Volunteering During COVID-19
We especially need volunteers to help out at our community-based COVID-19 testing and/or vaccination events!
Medical professionals who plan to volunteer only at COVID-19 events do not need to complete one of the above privileging forms. Instead, they should complete our Volunteer Medical Professional for COVID-19 Event form (as well as the volunteer application and waiver).
