To assist YCSHCNs with the healthcare transition, we are building community partnerships and using a family driven systems change approach, prioritizing the voices of teens and parents. In Year 1 we conducted needs assessments for parents and youth to identify barriers to the healthcare transition, connected families to medical homes and community resources, worked alongside other providers and community organizations to build systems of care across Mecklenburg County, and more.
