Dr Sveta Mohanan is a Board-Certified Family Physician, practicing full spectrum Primary Care in the Charlotte area since 2003.  She has focused her career on underserved populations, with particular interest in HIV, PrEP, Hep C and Women’s Health.  She went to medical school at Kasturba Medical College in India and completed her residency at Duke University Medical Center.  She has held academic appointments at UNC Chapel Hill and Wake Forest Atrium as Associate Professor.  She has taught resident doctors for over 20 years, focusing on Inpatient care, HIV care, IUD and Nexplanon insertion.  Her areas of expertise include HIV care, including ARV treatment with injectables, PrEP and cure of Hepatitis C. Dr Mohanan is a well-known regional speaker, most recently with Atrium Health and CCHC  She accepts all insurance sources, including uninsured patients and is located at the Wilkinson Boulevard location.  She works closely with the team at CCHC, including Dental, Behavioral Health and Integrated Social Work.  She is accepting new patients.     

 

She enjoys traveling with her husband, cooking and has a lifelong connection to the CCHC purpose of providing care for all.  She is happy to be part of the CCHC family and especially happy to partner with the Robin to her Batman, DeeDee Richardson.

Israel M. Mendez, MS, PA-C

Mendez is a graduate of East Carolina University where he earned a Bachelor of Science in Public Health with a concentration in Community Health. He was the distinguished graduate of the year and graduated Magna Cum Laude. He returned to East Carolina University to earn his Master of Science Degree in Physician Assistant Studies where he served as class president, taught medical Spanish, and volunteered at free clinics. Mendez then completed the Healthcare Management Certificate program from Vanderbilt University.

 

Prior to his PA studies, Israel accumulated valuable experience as a research assistant, certified nursing assistant, and clinic director for a non-profit. His research has been published in the Journal of the Medical Library Association and the Journal of Health Promotion Practice.

Before joining CCHC, Israel provided comprehensive prenatal, adult, and pediatric care at another FQHC, where he developed a deep understanding of the commitment required to care for patients and gained a profound appreciation for the unique needs of diverse communities. He looks forward to collaborating with the CCHC team to deliver high-quality care and support the growth and development of the organization. 
In his free time, he enjoys exercising, traveling with his wife, and engaging with his local church.

Dr. Canipe has served with Charlotte Community Health Clinic since 2017. Before coming to Charlotte, she worked for 3 years in Waco, TX in private practice, then for over a decade with Roanoke Chowan Community Health Center in Murfreesboro, NC. She enjoys improving health literacy, managing chronic disease in collaboration with patients, and encouraging preventative care.

Amanda Cruz first started at CCHC in 2019 as an FNP student, and came on as a Family Nurse Practitioner in 2020. Her 14+ years of experience include work as a nursing instructor, critical care nurse, sexual assault nurse examiner, and forensic nurse examiner for intimate partner violence cases. She is passionate about community and global health, and loves giving back by volunteering both in her community and abroad!

Nga Tran, MD

Dr. Tran is a Board-Certified Pediatrician who grew up in Charlotte and has moved back home to practice medicine. She has a passion for community/immigrant health and enjoys seeing kids of all ages. In her spare time, she enjoys spending time with her family and dog. She likes to travel and try out new restaurants with her husband.

Dr. Andrea S. Rice is proud to join the CCHC team as a pediatrician. She was born and raised in the Lowcountry of South Carolina and has lived in the Charlotte, NC area since 1999. She obtained her medical degree from the Medical University of South Carolina, completed her internship at the Medical College of Virginia and her residency at East Carolina University School of Medicine. Dr. Rice is board certified, a member of the American Academy of Pediatrics, and has been caring for patients for over 20 years. Prior to CCHC, while in Charlotte, she has had over 10 years of experience with community health at C.W. Williams Community Health Center and over 7 years of experience with rural health in private practice at Boiling Springs Pediatrics and Family Medicine. Her interests include preventative care and nutrition. She is excited to be a part of CCHC where she will strive to be her patient’s advocate. 

Dr. Harold Zeltner - MD

Dr. Zeltner received his medical degree from Albany Medical College in Albany, NY, and received his MBA from Union College in NY. He specializes as an Internist and possesses over four decades of medical experience, working in both private practices and health centers. After leaving his Internal Medicine private practice, he served as the Medical Director for the Center for Disability Services and then the Chief Medical Officer of the Whitney Young Health Center in Albany, NY.

 

As the Chief Medical Officer, Dr. Zeltner gained extensive experience in hospital and clinical settings. Further, he has developed strong expertise in working with minority and low-income populations. He balanced the optimization of medical/clinical services of the entire Health Center with his own part-time delivery of medical care, consistent with the demands of management duties and medical specialty. In addition to his medical career, Dr. Zeltner has spent multiple years on the Board of Directors of dramaturgical theaters in New York. He will primarily work at our West Clinic.

Amy Evans is originally from Chicago, IL. Before joining CCHC, she worked at a Federally-Qualified Health Center in Chicago for five years. Upon moving to Charlotte in 2019, she worked at the Mecklenburg County Health Department in the STI, Family Planning, and TB Clinics. Amy has a passion for community health and serving vulnerably populations, so she is excited to return to a primary care setting. Her interests include reproductive justice, social determinants of health, and health equity – that quality health care should be a right, not just a privilege.

Rachael grew up in Charlotte and recently moved back after spending a few years in Boston, MA, where she worked at another FQHC. She is inspired by the belief that healthcare is a right and not a privilege. She is also passionate about removing barriers to great care, and she hopes to add to the welcoming environment already established here at CCHC. She is particularly interested in destigmatizing mental health. In her free time, she enjoys outdoor activities and art projects!

Heather is a Licensed Clinical Social Worker with over 5 years of experience. Before joining CCHC, Heather worked at Roof Above – previously Urban Ministries – as a Social Worker, Therapeutic Art Coordinator, and Therapeutic Housing Navigator. In this role, she primarily worked with with individuals experiencing homelessness. She specializes in an understanding of clients with severe and persistent mental illness, passionate about walking with clients on their personal path toward wellness, advocating for those who cannot access the care they need, and honoring the dignity of every person’s story.
